I've just noticed it all goes horribly wrong if you use -vf scale with a final 
x and y resolution which are not multiples of 16 - you get lots of alignment 
errors.

Also - if the audio is at 32kHz it goes wrong, but this can be corrected by:
-ao alsa -af resample=44100 (just tested this)
